Self-Help Legal Services has grouped required forms for many common procedures into a digital form packet for your convenience.
Once you complete the online coversheet which is the first page of the form packet, the digital packet will automatically populate your contact information into the forms.
Please note: Filling in the coversheet only partially completes your forms.
Accessing the Packet:
- Click the "Family Law Form Packets" below, and select the appropriate form link(s). This will open up the packet.
- Save or Download the packet to your computer first.
- Once you have saved it, please close it out all the way.
- Go back into your computer to locate the packet and begin filling out the packet to the best of your abilities.
- Please make sure to save it frequently as you go through the packet.
- If there is anything on the packet you don’t understand, you may skip it and complete everything else.
- When you are done, please save it again to your computer.
